The prognosis is dependent on the histology and stage.
With spreading to cervical lymph nodes, the disease-specific survival is halved relative to the same T-stage without spreading. Increasing N-category also gives increasing risk for distant metastases.
Adenoid cystic carcinomas has a special disease course with late occurring distant metastases, which grow slowly. The patient may live long with the disease.
Confirmed distant metastasis at present usually eliminates curative treatment.
